I understand that my child must be free from COVID-19/flu-like symptoms to attend.

If during the course of the day, any of the following symptoms appear, I agree to have my child picked up from the School within one hour. I have arranged for someone to be available during School hours for this purpose.

Symptoms include:

feeling feverish or a measured temperature greater than or equal to 100.0 degrees Fahrenheit
cough
shortness of breath
chills
loss of taste or smell
sore throat
muscle aches
headache
diarrhea
runny nose
congestion

While we understand these symptoms can also be related to non-COVID-19 health issues, we must proceed with an abundance of caution during this time. Symptoms typically appear 2-7 days after being infected, so please take them seriously.

I agree that my child will need to be symptom-free and fever-free (without the use of fever-reducing medicine) for 24 hours before returning to the facility.

I understand that my child’s temperature will be taken prior to or upon arrival and I will be asked screening questions daily. I also understand that my child’s temperature will be taken periodically throughout the day while at school.

I understand that my child will wear a cloth face covering while at school as written in the School’s Mitigation Plan.

I understand that the School has adopted enhanced cleaning procedures and I will comply with any required procedures that the School requests of me personally.

I understand that in an effort to minimize the number of individuals entering or departing the campus at any given time, I will follow the School’s procedures for drop off and pick up. This includes the observance of assigned staggered drop off and pick up times as applicable.

I understand that my child will be asked to wash their hands upon arrival and throughout the day and may need to use hand sanitizer or sanitizing wipes if necessary.

I understand there are certain activities that require staff to have physical contact with my child. These include, but are not limited to, responding to any health and safety needs of the child.

I understand that I must immediately notify the School if I become aware of any person, with whom I or my child have had contact, who:

exhibits any of the symptoms listed above,
is advised to self-isolate,
is advised to quarantine, or
has tested positive or is presumed positive for COVID-19

I understand that while my child is at the School they will come into contact with other children and staff and that no restrictions, guidelines, or practices will remove 100% of the risk of exposure to COVID-19, as the virus can be transmitted by persons who are asymptomatic.

I understand that the circumstances surrounding this pandemic could change at any time, and changes to any policies or procedures will be communicated to families in a manner as timely as possible.

I understand that recess times will be different from previous years.

I understand that social distancing measures will be used in the classroom including:

an individual workspace
there will be less free movement around the classroom
plexiglass dividers will be used when close contact cannot be avoided
physical contact will be replaced with air hugs, elbow bumps, etc.

I understand that my child needs to bring a large refillable water bottle to school each day.

I understand that my child needs to practice wearing and safely removing a face-covering prior to their attending school in person.

I understand the need to support my child’s positive compliance with the requirement of wearing a face covering and washing his/her hands frequently.

I understand that our family’s observance of health and safety protocols outside of school (e.g. staying home as much as possible, wearing face coverings, avoiding places and activities that may lead to possible exposure to the virus, staying home when sick) will help keep the on-campus staff and students safe.

I understand that I need to report my child’s absence on the School’s absentee line.
